    • A laparotomy is a surgical procedure involving a large incision through the abdominal wall to access the abdominal cavity.

    • A laparotomy set is a collection of instruments needed to perform such open abdominal surgeries — for inspection, retraction, hemostasis (controlling bleeding), cutting, suturing, etc.

    • The “27 PC” indicates 27 individual instruments (plus perhaps a tray), enough to cover the major functionalities needed.

    • A proctoscope is a rigid instrument (tube) used to examine the anal canal and lower rectum.

    • The “small” size refers to a narrower/smaller diameter and often shorter length, used especially when anatomy requires it (children, discomfort, narrow canal, etc.

A small proctoscope usually has these components:

      PartDescription
Outer Tube (Speculum / Barrel) Rigid hollow tube, open at both ends. One end enters the anus/rectum; the other allows vision/light/instrument access.
Obturator / Introducer A smooth (“olive‑shaped” or conical) insert that snugly fits inside the tube during insertion to reduce discomfort; removed after insertion.
Light Source / Lamp Holder Some have built‑in fiber optic/light guide or lamp holder to attach external light source; some are translucent/disposable to allow light via external source.
Viewing Window / Edged/Cut-out Some versions have a cut‑out window or “half‑cut” or “full‑cut” to allow side‑viewing or instrument passage; small size sometimes has smaller or no window.
Handle / Grasping Portion Either a flange or flat handle flange that allows holder/manipulation. Might also have features for securing light adapter.
Small proctoscopes may be made of:

      • Stainless steel (reusable type) — durable, autoclavable, good optics when polished.

      • Brass (older models) often with nickel or chrome plating — less common now.

      • Plastic / Polystyrene / Transparent medical‑grade plastic — often disposable to reduce risk ofinfection.

      • Materials are generally latex‑free (especially disposable/private use) to avoid allergies.

        • Indications (When To Use a Small Proctoscope)

A small proctoscope is used in situations like:

          • Anal canal / rectum examination for bleeding, pain, discharge, pruritus (itching).

          • Diagnosis of hemorrhoids, fissures, polyps, proctitis or other lesions near the lower rectum.

          • Taking biopsies of small lesions in the anal canal or lower rectum.

          • Follow‑up or screening in cases where a full‑size scope is uncomfortable or not needed.

          • Paediatric patients, or in patients with stricter anal canal, in whom large scopes are painful/difficult.

          Uses / How It’s Used

          Steps / uses:

          1. Preparation – patient positioning (usually Sims’/left lateral), lubrication of scope, ensuring light source ready.

          2. Digital Rectal Examination – to feel for obstruction, masses, etc., before using proctoscope.

          3. Insertion – with obturator in place, smooth rounded tip, sliding in slowly. Once in, remove obturator.

          4. Illumination & Inspection – view mucosa inside anal canal & lower rectum. Look for bleeding, lesions, ulcers, polyps, hemorrhoids.

          5. Intervention (if needed) – biopsy, excision of small polyps, cauterization, etc., can be done via small opening / with instruments through window.

          6. Withdrawal – inspect on way out; ensure no lesion missed; monitor for bleeding.

An Olsen‑Hegar is a combined instrument: it works both as a needle holder (to grip suture needles) and has built‑in scissors (for cutting sutures) in one tool. This saves instrument changes during suturing.

        Indications (When It’s Used)

The Olsen‑Hegar 5.5″ needle holder is used typically in:

        • Holding suture needles securely during suturing in surgical procedures.

        • Cutting suture material immediately after a stitch without needing to switch to separate scissors (because scissors built in).

        • Procedures requiring medium‑weight sutures and needles (neither extremely fine nor extremely heavy). Some versions are used with sutures in the range like 2‑0, 3‑0, 4‑0, etc. Common in general surgery, minor surgery, gynecology, dentistry, plastic surgery, etc. Wherever precision together with some speed is needed.

        • Uses & How It’s Used

          Typical workflow / usage:

          1. Surgeon loads needle into the jaws (held firmly via cross serrations or TC inserts).

          2. Using the finger rings and ratchet lock to steady the holder, passes the needle through tissue layers.

          3. Once suture is secured, instead of switching instrument, uses the built‑in scissors section (just below the jaw) to cut suture tail.

          4. Releases the ratchet; clean instrument; repeat for next stitch etc.

          5. After use, instrument must be cleaned thoroughly and sterilized (autoclave or other hospital‑approved method).
